About the Course:
This is a modular course and has been developed to assist Maintenance Error Investigators to determine the root cause of maintenance errors within the workplace. Attendees will gain the necessary competence to confidently conduct MEDA investigations that will greatly contribute to the Approved Maintenance Organisations’ safety and efficiency standards.

Utilising the Boeing Maintenance Error Decision Aid (MEDA) tool, attendees will be given an opportunity to enhance their interview skills including the cognitive interview, funnelling and conversation management. Practical exercises will enable attendees to gain confidence in their newly acquired skills and knowledge as well as provide a means of assessing competence and aptitude.

This course will:

  • Define the role of a Maintenance Error Investigator in the aircraft, engine or component maintenance environment and develop personal competence to develop the organisation’s error management system into a value adding process
  • Develop attendees’ ability to confidently conduct error investigations
  • Demonstrate proven police interview techniques and integrate best practice in a flexible environment
  • Provide ample opportunity for students to practice the interview techniques in a realistic and challenging manner
  • Ensure organisations’ compliance with the EASA Part 145 requirements
  • Develop the attendees’ ability to risk assess MEDA solutions for the maximum return on investment from their mandated EASA error management programme
  • Develop skills in preparing, opening, conducting and closing interviews, dealing with difficult situations and difficult people

Who Should Attend?
This is a specialised course ideally suited to:

  • Prospective maintenance error investigators selected from the Part 145 maintenance system
  • Quality Assurance staff, engineers and operators
  • Managers and those responsible for developing their company’s internal occurrence management systems to comply with the EASA Part 145 Regulations

It would also be of benefit to:

  • Regulatory Authority inspecting staff

Course Content:
  • An overview of Human Factors and error management specifically error, violation and defences
  • An in-depth look at the Boeing Maintenance Error Decision Aid (MEDA) investigation tool
  • Interview skills:
    – Questioning techniques
    – Interview preparation
    – Opening / conducting / closing interviews
    – Dealing with difficult situations in the interview
    – Who should conduct interviews?
    – Interview techniques, such as “Timeline / funnelling”
    – Dealing with challenging interview situations
    – Collection of suitable evidence
  • Extensive investigation scenario practical exercises
  • Analysis of results / evidence and risk assessment of findings
  • Importance of a database, trend analysis, and feedback
  • Developing an organisational culture in which error reduction can flourish

It would be of benefit, but not essential, if the attendee had attended an EASA Part 145 compliant Human Factors training course.

About the Instructor:

Mr. Peter Jewell, Director, HOFTCO Pty Ltd 
Peter achieved his license as a Crew Resource Management Instructor (CRMI) with the UK CAA in 2003. With a background as aircrew on business jet aircraft, he later went on to ground and simulator (UK CAA) instructing for FlightSafety International. He also has a Certificate IV in Workplace Assessment and Training.

Peter has delivered CRM / Human Factors training to Management, Flight Deck Crew, Cabin Crew, Engineering and Operational teams including Train-the-Trainer (Instructor) Courses. He has delivered training to over forty operators in fifteen countries to date.

Peter had previously been involved in training since the early 1980s in both maritime and aviation safety critical activities.
